Perforation of benign peptic ulcer in hiatal hernia into the pericardium, resulting in pneumopericardium.

چکیده
A 57-year-old woman with a medical history of hiatal hernia, peptic ulcer disease, alcoholism, and schizophrenia was admitted to the emergency department with epigastric pain and fever. The patient had not been taking her prescribed proton pump inhibitors. A peptic ulcer perforation (1.5cm) in the hiatal hernia sac was discovered during gastroscopy, and an attempt to close the perforation with metal clips was unsuccessful (●" Video 1). The patient’s general state became more severe following gastroscopy. Tachycardia with atrial fibrillation, dyspnea (saturation <80%), and hypotension (systolic blood pressure <60mmHg) occurred. The patient was referred for surgery with suspicion of pneumopericardium (●" Fig.1). A gastrotomy was made during laparotomy. A fistula to the pericardium was located and directly sutured. A tissue sample was taken from the ulceration. A cardiothoracic team performed pericardial drainage, which was complicated by the rupture of the fragile, inflamed wall of the right ventricle. Suturing of the right ventricle through the sternotomy was performed. The patient died 36 hours later due to multiorgan failure. Massive fibropurulent pericarditis was diagnosed in the autopsy. Specimens from the ulceration showed only fibrosis and inflammation with no trace of malignancy. Peptic ulcer perforation located in the hiatal hernia sac with pneumopericardium is a very rare complication of benign stomach ulceration. Typically, this complication is related to malignant neoplasms of the stomach located in a hernia sac or in the fundus with invasion of the diaphragm and pericardium or as a complication after Nissen fundoplication [1]. Salling et al. described a case of pneumopericardium related to a gastric ulcer in a hiatal hernia, and cited seven similar cases from the literature [2]. The mortality rate is very high and it demands a multidisciplinary approach as well as a quick surgical response. Currently, there are no published guidelines or standards on how to manage such a complication [3,4]. We suggest that in the case of a gastropericardial fistula, a computed tomography scan of the chest and abdomen with administration of oral contrast should be performed instead of endoscopy, which could dramatically worsen circulatory insufficiency. Management of oesophageal perforation associated with benign stricture and hiatus hernia by oesophagoplasty and fundoplication.
The operation of oesophagoplasty and Nissen fundoplication has proved valuable in the management of intractable peptic stricture of the oesophagus. This report concerns its use in perforation of the oesophagus in an 81-year-old woman with benign peptic stricture in whom perforation occurred during instrumentation. She made an uneventful recovery from operation and remained well and able to swal...
متن کاملPenetration of the left ventricular myocardium by benign peptic ulceration: two cases and a review of the published work.
Two cases of penetration of the left ventricular myocardium by benign peptic ulcer are reported. Twenty five similar cases in the world published work are reviewed. The condition is only possible when there are fibrous adhesions between the stomach and diaphragm and the pericardium. In addition, the left lobe of the liver may be small. Alternatively, an ulcer within a hiatus hernia may erode in...
متن کاملSudden death from perforation of a benign oesophageal ulcer into a major blood vessel.
Two cases of sudden death due to perforation of a benign oesophageal ulcer into a major blood vessel are reported. In one man, anaemia and aspiration pneumonitis dominated the clinical picture. He had an oesophageal stricture and a chronic peptic ulcer associated with an incarcerated hiatus hernia. Death was due to haemorrhage caused by perforation of the ulcer into the thoracic aorta. The seco...

The presence of free air within a hiatal hernia sac is an uncommon reported entity. This finding has been described in the surgical literature1-4 secondary to incarcerated hiatal hernias complicated by gastric ulceration and perforation. There have, however, been no reports of this entity in the radiology literature. Our patient presented to the emergency department complaining of epigastric pa...

A 88-year-old male patient presented with fever, singultus and retrosternal pain. After 8 days of antibiotic therapy not resulting in clinical improvement, he suddenly developed a pneumopericardium. Contrast swallow and endoscopy showed intrapericardial perforation of a benign gastric ulcer. Excision of the ulcer and suturing of both the stomach and the diaphragm as well as lavage of the perica...
